If either Godparent is from a church outside the West St. James Cluster, they must provide a pastor's letter showing membership. Intended Godparents who live away and choose to attend a Baptism seminar in their home parish must provide a letter from that parish signifying their attendance at a seminar.

Please fill out this form. When you submit the application it will be emailed to the Pastoral Center. Please call the Pastoral Center the week prior to the scheduled Baptism to make sure we have all of the information for your certificate and to verify that you will be at the Baptism that weekend.
